Parish Census Form --An individual must fill in this form and return it to the parish office in order to be a registered member of St. Jude Thaddeus.

Newcomer Survey --So, you're joining our parish? We want to make you feel welcome and know how we can best meet your needs. Please fill in this form along with the Parish Census Form and return both to the parish office.

Universal Fieldtrip And/Or Youth Activity Release Form --This form is necessary for any youth to participate in activities off parish grounds. Please fill in completely and return with parent signature. If the Youth Office has a current copy of your health insurance card another copy is not necessary.

RCIA Registration Form --Do you have a friend or family member who is not Catholic but is curious about our faith? Perhaps you would like to learn more about the Catholic faith yourself? The Rite of Christian Initiation of Adults is the answer to these questions! RCIA sessions begin in September. Please print the registration form and return to the parish office.
